Do you understand the animal’s ownership resides with Pet Harbor while you are fostering?

Fostering is a temporary situation. Do you understand that fostering is different from adoption and that you cannot permanently keep fostered pets unless you go through the adoption application process the same as any adoption applicant?

Are you willing to meet potential adopters at adoption events and other reasonable times prearranged with you to show the foster pets?

Are you willing to make sure the pet is clean and bathed prior to a showing? (Pet Harbor gets donations of shampoo and food occasionally and will happily give all such items to foster homes as we get donations. Unfortunately, at this time we cannot reimburse monetarily for grooming or other expenses incurred other than preapproved medical expenses.)

Are you willing to work with foster pets on training/disciplining?

By indicating yes and submitting this form, I signify that all information given in this application is true and correct, to the best of my/our knowledge and that I am of legal age (minimum 18 years) to enter into a contract. And by indicating yes and submitting this form, I hereby further grant consent to Pet Harbor and its volunteers to seek information about the members of our household from all named and unnamed sources within this application, including but not limited to veterinarian, rental agent, personal references, to assist in the application process. Your interest is deeply appreciated because every Siberian Husky we can place in a foster home is one less that is homeless, hungry, abused, or killed. PET HARBOR WOULD LIKE TO THANK YOU FOR TAKING THE TIME TO COMPLETE THIS APPLICATION. Secondhand pets give first-class love.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. This foster care Agreement ("Agreement") is entered into between Pet Harbor Rescue & Referral, Inc. (the "Rescue" or "we" or "us"), a Virginia corporation, and the undersigned (the "Caretaker" or "you"). In consideration of the mutual promises in this Agreement and other good and valuable consideration, the Rescue and Caretaker agree to the following terms and conditions, intending to be legally bound:

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. The Pets. We will give you dog(s) or cat(s) ("Pets") for temporary foster care and boarding (together the "Foster Care") for an indefinite period, until adoption unless the Pets are returned to us earlier as provided in "Liability" below.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement.

You will provide Foster Care in your home at the address in the foster application subject to the terms and conditions of this Agreement.

Your Foster Care for the Pets will be comprised of the following:

- Providing daily wholesome dog or cat food and water

- Providing a clean grassy area for elimination for dogs and/or a clean litter box for elimination for cats

- Providing daily outdoor exercise for dogs

- Bottle feeding for puppies or kittens, if necessary

- Providing a warm, safe place inside your home

- Administering medication, if any

- Transporting the Pets to/from pre-approved veterinarians, if necessary

- Socializing and playing

- Cleaning, bathing

- Transporting the Pets to/from adoption showings

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Veterinary Care. You will use Pet Harbor-approved veterinarians for the Pets. You will telephone the Rescue at the number you will be provided to advise of any emergencies involving the Pets, for preapproval of veterinary care.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Vaccinations. We will provide you a copy of the rabies certificate and a summary sheet for the Pets describing the medical care and vaccinations the Pets have had in our care. The complete medical records for the Pets will be maintained by the Rescue. You agree to take the Pets to any scheduled veterinary appointments.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Costs and Expenses. The Rescue will pay all veterinary treatment costs that may be incurred for the Pets during the Foster Care provided that we have given you and the veterinarian prior approval for each such treatment. Caretaker costs are not reimbursed, but items donated to the Rescue, such as food, formula, bowls, towels, crates, shampoo, will be provided to Caretaker as donated. Upon termination of this Agreement, all remaining items will be returned to the Rescue. You are providing the Foster Care out of your love for dogs and as a volunteer, at no charge to the Rescue.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Policies. You will abide by the mission, rules, regulations, policies, and programs of the Rescue while you engage in Foster Care. You understand and agree that the Rescue may refuse foster applications for any reason. You consent to Pet Harbor visiting your home from time to time to observe the animals and their living quarters. You also agree to all provisions in and have signed this Agreement.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Liability. You are not liable to us for any injuries to, illness, or disappearance of the Pets arising out of the Foster Care except if such injuries, illness, or disappearance are caused by or arise out of your gross negligence or intentional misconduct. We are not liable for any bodily injury or property damage, liabilities, losses, judgments, or injuries whatsoever to you or other persons or to your or another person's animals and pets caused by the actions, behavior, or health of the Pets, or arising out of the Foster Care except if such damage, liabilities, losses, judgments, or injuries are caused by our gross negligence or intentional misconduct. You assume the risks of being bitten, scratched, injured, or frightened by cats, kittens, dogs, and puppies in connection with Foster Care for the Rescue. You will indemnify, defend, and hold the Rescue harmless from and against any claims, lawsuits, injuries, damages, losses, costs, or expenses whatsoever sustained by any companion animal or any person in connection with your intentional misconduct or grossly negligent performance of Foster Care for the Rescue or your breach of the Rescue's rules, regulations, policies, and programs. 3.Returning Pets. You agree to contact us immediately in the event that you are no longer able or willing to provide Foster Care to any or all of the Pets. You will continue to care for the Pets for a reasonable period until we are able to retrieve Pets being returned or until we can accept the return of the Pets from you. You will work with us to promptly arrange mutually satisfactory times to bring the Pets for meetings with prospective adopters. You may only entrust the care of the Pets to another person or relocate the Pets with prior approval of the Rescue.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Adoption by Caretaker. In the event that you wish to adopt any or all of the Pets as household pet(s) for yourself, you must enter into the usual adoption process and will be given priority consideration over other applicants only if no other applications for said pet are pending.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Loaned Equipment and Supplies. Any equipment or supplies temporarily provided to you for use in providing the Foster Care will be returned in good, clean condition (subject to normal wear and tear) at the end of the Foster Care Agreement. If the items are lost, stolen, or damaged beyond normal wear and tear as a result of your negligence or misconduct, you will promptly pay the replacement price.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Cruelty Statement. No member of your household has ever been convicted of animal neglect, abandonment, or cruelty.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Laws. The Caretaker will comply with all national, state, and local regulations concerning keeping a pet, including, but not limited to, zoning regulations regarding numbers of animals allowed per home of the jurisdiction in which you reside and any rules set forth by the owner of the home if the owner is other than the Caretaker.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Entire Agreement; Modifications; Binding Effect. This Agreement is the entire Agreement between you and us. Any modification to this Agreement must be in writing, signed by both you and us. This Agreement binds your and our respective heirs, executors, representatives, successors, and assigns. You have accurately and truthfully completed this volunteer application and Agreement. You have read, understand, and agree to the Guidelines for Foster Homes.
