Please understand that if you think your dog is "better off" somewhere else, that is not the case. You already love your dog. Someone new will never love him or her like you do. And everything will change in his or her world. His/her people, environment, and name all will change. Everything he or she knows will vanish. Your dog is NOT "better off" somewhere else. Dogs don't care how big the couch is or the yard is or whatever it is causing you to end your relationship.
Please understand also you won't know details about your dog's future. Once you sign him or her over, that's the end.
If you change your mind and need help fixing whatever is causing you to think he or she will be "better off" somewhere else, we're happy to try to help! Before you fill out this form, please be sure you really want to surrender your dog.
PLEASE BE HONEST. Lying does not help your dog.
The procedure is:
1. Pet Harbor's intake volunteers review this form.
2. Send the vet records to us, preferably via email, including, at a minimum, current rabies CERTIFICATE, medical and vaccine history, spay/neuter information, and the recent results of a heartworm test (must be within 45 days prior to the date of surrender).
3. We will arrange for you to bring the dog to the nearest volunteer to do a temperament evaluation.
4. We then will let you know if your dog can come into our program and will work out those details with you.
If this is for more than one dog, you must do a separate form for each dog.
