Dog Foster Application Immediately Prior to Adoption

Has anyone in your household ever been convicted of an animal-related charge?

Do you understand this application governs the foster period immediately prior to adoption and will no longer apply beginning with the adoption date designated in the adoption paperwork?

Do you understand that if at any point during the foster period, you return the dog, (a) the paperwork will all be destroyed as if you had not fostered or planned to adopt, (b) donation received returned to you, and (c) there will be no negative consequences associated with returning the dog, as there might be when returning an adopted dog, such as, but not limited to, being placed on a "Do Not Adopt" list?

By indicating yes and submitting this form, I signify that all information given in this application is true and correct, to the best of my/our knowledge and that I am of legal age (minimum 18 years) to enter into a contract. For purposes of this form, "Pet Harbor," "the Rescue," or "we" refers to Pet Harbor Rescue and Referral, Inc.; "you," "the Caretaker," or "Foster family or parent" refers to the members of the family fostering the dog; "the Dog" refers to the fostered pet. PET HARBOR WOULD LIKE TO THANK YOU FOR TAKING THE TIME TO COMPLETE THIS APPLICATION.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. The Dog. Pet Harbor will give you a dog for temporary foster care and boarding (together the "Foster Care") for a maximum period of time mutually agreed upon in advance, usually one week.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Veterinary Care. Unless prearranged and preauthorized by Pet Harbor administrator(s), Pet Harbor will not cover or reimburse any medical expenses incurred for the dog during the foster period.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Policies. You will abide by the mission, rules, regulations, policies, and programs of the Rescue while you engage in Foster Care. You also agree to all provisions in and have signed this Agreement.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Liability. You are not liable to us for any injuries to, illness, or disappearance of the Dog arising out of the Foster Care except if such injuries, illness, or disappearance are caused by or arise out of your gross negligence or intentional misconduct. We are not liable for any bodily injury or property damage, liabilities, losses, judgments, or injuries whatsoever to you or other persons or to your or another person's animals and pets caused by the actions, behavior, or health of the Dog, or arising out of the Foster Care except if such damage, liabilities, losses, judgments, or injuries are caused by our gross negligence or intentional misconduct. You assume the risks of being bitten, scratched, injured, or frightened by the dog in connection with Foster Care for the Rescue. You will indemnify, defend, and hold the Rescue harmless from and against any claims, lawsuits, injuries, damages, losses, costs, or expenses whatsoever sustained by any companion animal or any person in connection with your intentional misconduct or grossly negligent performance of Foster Care for the Rescue or your breach of the Rescue's rules, regulations, policies, and programs. Returning Dog. You agree to contact us immediately in the event that you are no longer able or willing to provide Foster Care to the Dog. You will continue to care for the Dog for a reasonable period until we are able to retrieve the Dog being returned or until we can accept the return of the Dog from you. You may only entrust the care of the Dog to another person or relocate the Dog with prior approval of the Rescue.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Cruelty Statement. No member of your household has ever been convicted of animal neglect, abandonment, or cruelty.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Laws. The Caretaker will comply with all national, state, and local regulations concerning keeping a pet and any rules set forth by the owner of the home if the owner is other than the Caretaker.

By indicating yes and submitting this form, I also signify my understanding of and agreement with the following paragraph of the foster agreement. Entire Agreement; Modifications; Binding Effect. This Agreement is the entire Agreement between you and us. Any modification to this Agreement must be in writing, signed by both you and us. This Agreement binds your and our respective heirs, executors, representatives, successors, and assigns. You have accurately and truthfully completed this foster application and Agreement.
